Job Description

Tl’azt’en Nation is seeking a caring, reliable, and community minded Outreach Worker to join our Social Development Department in Tache, BC.

This is a meaningful front line role for someone who wants to make a real difference in the lives of community members. The Outreach Worker will support individuals and families accessing Income Assistance, Adult In Home Care, and related community services. This position helps connect people to the supports they need, promotes stability and wellbeing, and plays an important role in strengthening community care.

This is an in community position. The successful candidate will be required to report to site daily, as the role involves direct support to clients, families, staff, and service providers.

About the Role

The Outreach Worker will provide practical, respectful, and culturally appropriate support to community members. This includes helping clients identify needs, access services, develop support plans, navigate systems, and connect with health, housing, employment, social, and wellness resources.

Duties and Responsibilities

Provide direct outreach support to community members receiving Income Assistance and Adult In Home Care services.

Work with clients to identify needs and develop individualized support plans with short term and long term goals.

Support clients in accessing health,



social, housing, employment, wellness, and community services.

Advocate for clients and help them navigate systems, forms, appointments, referrals, and available supports.

Coordinate and support the Adult In Home Care Program, including homemaking services, file management, seasonal cleaning coordination, and service tracking.

Build robust working relationships with community programs, agencies, service providers, and informal support networks.

Deliver wellness and health education outreach based on community needs.

Prepare intake forms, assessments, progress notes, service reports, and other client documentation.

Maintain accurate, timely, and confidential client files.

Monitor support plans to ensure services remain appropriate and responsive.

Participate in case conferences, staff meetings, and interagency planning as needed.

Identify service gaps, emerging needs, and barriers affecting clients and families.

Support community based initiatives that promote wellness, independence, safety, and self sufficiency.

Perform other related duties as assigned by the Social Development Manager.

Qualifications

Post secondary education in Social Development, Human Services, Social Work, Health, or a related field, or an equivalent combination of education and experience.

Experience providing outreach,



advocacy, client support, or community based services.

Ability to support individuals and families facing social, health, housing, financial, or personal challenges.

Strong interpersonal, communication, and relationship building skills.

Ability to maintain confidentiality and handle sensitive information with care.

Experience with client documentation, file management, and basic reporting.

Knowledge of community resources, referral processes, and support services.

Basic proficiency with Microsoft Word, email, and computer systems.

Knowledge of Tl’azt’en Nation culture and community, or willingness to learn.

Valid BC Class 5 Driver’s Licence.

Satisfactory Criminal Record Check.
