About the Opportunity

We are seeking an experienced and compassionate Nurse Practitioner (NP) to provide primary and transitional care to older adults across two retirement residence locations in Bolton and Brampton.

This is a primarily clinical and client-facing role, focused on comprehensive assessment, diagnosis, treatment, medication management, care coordination, and supporting older adults with complex health needs.

The NP will work collaboratively with clients, families, caregivers, nursing staff, physicians, and other members of the circle of care to develop and implement individualized care plans. The role will also provide clinical leadership and mentorship to the Transition Care team.

This is an excellent opportunity for an NP with experience in geriatric, primary, community, transitional, or post-acute care who is looking for a flexible permanent part-time position.

Key Responsibilities
Clinical Care

- Conduct comprehensive physical, mental, and functional health assessments.
- Assess, diagnose, and manage acute and chronic health conditions in older adults.
- Develop, implement, and evaluate individualized care plans.
- Provide pharmacological and non-pharmacological treatment within NP scope of practice.
- Order and interpret laboratory tests, diagnostic imaging, and other investigations.
- Complete medication reconciliation and collaborate with pharmacy partners.
- Monitor client progress and modify treatment and care plans as required.

Transitional Care & Care Coordination

- Support safe transitions from hospital to community-based care.
- Collaborate with hospital discharge teams and community healthcare providers.
- Participate in care conferences with clients, families, caregivers, and the interdisciplinary team.
- Identify clients at risk of deterioration or hospital readmission and implement appropriate interventions.
- Coordinate care to help clients maintain safety, independence,



and optimal health outcomes.

Client & Family Support

- Provide education regarding health conditions, medications, treatment plans, and self-management.
- Promote health maintenance, disease prevention, and functional independence.
- Engage clients and families in shared decision-making and care planning.
- Build strong, trusting relationships with clients and their families.

Clinical Leadership & On-Call Support

- Provide clinical guidance and mentorship to nursing and Transition Care staff.
- Provide remote clinical support by phone or virtually during assigned availability periods.
- Respond to clinical concerns and support appropriate escalation when required.
- Conduct in-person assessments when clinically necessary.
- Provide clinical education to staff as required.
- Participate in quality improvement and medication safety initiatives.

Documentation & Quality

- Complete timely and accurate clinical documentation in accordance with CNO standards.
- Maintain clear communication with clients, families, and the interdisciplinary team.
- Complete required assessments and documentation, including LTC application assessments where applicable.
- Support infection prevention, health and safety, accreditation, and quality improvement initiatives.
- Report adverse events and contribute to a culture of client safety.

Schedule & Coverage

This is a permanent part-time position of approximately 14 hours per week, generally distributed across 2 days per week, with approximately one day at each location.

The position includes:

- On-site clinical assessments, care, and rounds.
- Documentation and care coordination.
- Interdisciplinary collaboration.




- Pre-scheduled availability/on-call periods to provide clinical guidance and support urgent client needs.
- Flexibility based on client acuity and program requirements.

Qualifications

- Registered Nurse (Extended Class) in good standing with the College of Nurses of Ontario (CNO).
- Completion of an accredited Nurse Practitioner program.
- Master's degree in Nursing.
- Minimum 2–3 years of recent Nurse Practitioner experience, or 3+ years of relevant clinical healthcare experience.
- Experience working with older adults, geriatric populations, or individuals with complex health needs is strongly preferred.
- Experience in primary care, community care, transitional care, post-acute care, or rural health is an asset.
- Experience with chronic disease management and complex care.
- Mental health, public health, and health promotion experience considered an asset.
- Experience working with infectious diseases.
- Knowledge or experience in program planning, evaluation, and data management/analysis is an asset.
- Current BCLS/ACLS and valid First Aid & CPR certification.
- Strong clinical assessment, communication, documentation, and decision-making skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
