The Barrie Community Health Centre is seeking a full-time permanent Community Health Case Coordinator to join our interprofessional team.

What you will do:

As a member of the Mental Health and Wellness Team, you will empower individuals and families to improve and manage their own health and wellbeing.

You will work with clients to meet their needs related to mental health, housing, social assistance, family support, immigration and adaption, income, and food security. You will also provide systems navigation, advocacy, short-term case management and supportive counselling as well as facilitate group programs.

What you bring to the role:

- A Bachelor of Social Work degree
- A member in good standing with the College of Social Workers and Social Service Workers
- 2-3 years of experience in a community or primary care setting
- Demonstrated group facilitation, interpersonal and organizational skills
- Demonstrated ability to work in an interdisciplinary team environment




- Understanding and ability to work from a strength-based and anti-discriminatory/anti-oppressive framework supporting the principles of Diversity, Equity and Inclusion
- Ability and interest to work with populations across the lifespan including youth, adults and seniors
- Proficiency in the use of computers and various software applications.
- A valid “G” driver’s license, relevant insurance and access to a vehicle
