Job Overview
We are seeking a compassionate and dedicated Community Health Worker to provide essential support and assistance to an individual in his home. The ideal candidate will possess a strong commitment to enhancing his quality of life through personalized care, empathy, and professionalism. As a Community Health Worker, you will deliver a range of services including personal care, medication management, and household support, ensuring safety, dignity, and independence.
Responsibilities
- Assist with daily activities such as bathing, dressing, grooming, and mobility to promote comfort and independence.
- Provide medication administration in accordance with prescribed schedules and safety protocols.
- Meal preparation and nutritional needs.
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a secure living environment.
- Offer companionship and emotional support to foster social engagement and mental well-being.
- Collaborate with healthcare professionals to implement care plans effectively and report any changes in client condition.
- Assist with mobility and transfer techniques, including heavy lifting when necessary, ensuring safety for both clients and caregivers.
- Maintain accurate documentation of services provided, observations, and any incidents in compliance with organizational policies.
Requirements
- Prior experience in assisted living, nursing home settings,
or home care environments is highly desirable.
- Knowledge of patient care practices including medication administration and hygiene assistance. Must have knowledge of ostomy and catheter care.
- Ability to perform heavy lifting safely while adhering to proper techniques. Must have knowledge of ceiling track lifts
- Strong caregiving skills combined with excellent communication and interpersonal abilities.
- Basic understanding of cleaning procedures and household management within a caregiving context.
- Relevant certifications or training in nursing assistance or caregiving are preferred but not mandatory.
Working hours are 7a - 7p, 7p - 7a and/or 7a - 930a. The position is casual, weekend availability is an asset and then a commitment to set shifts starting in September.
