Abbotsford Family & Friends Caregivers Support Coordinator

JOB SUMMARY: The Abbotsford Family & Friends Caregivers Support Coordinator oversees the operation and delivery of the Caregiver Support program. This position provides support groups, information, assistance, training, and referral to a variety of services for caregivers of older people. The Family & Friends Caregiver Support program is designed to support informal caregivers caring for an older adult.

· Full Time – 35 hours per week

· Hybrid – Home, office, community

· Location – Abbotsford

· Vacation- Paid vacation in accordance with organizational policy

YOUR ROLE:

· Exhibit a friendly and courteous manner

· Provide services in accordance with Abbotsford Association of Healthy Aging`s policies and procedures

· Maintain client, caregiver, and volunteer confidentiality

· Coordinate, facilitate, and schedule Caregiver support groups

· Coordinate events and presentations with outside agencies on caregiver topics

· Work with individual caregivers on locating, obtaining, and using available services and support in the community

· Provide opportunities for individual caregivers to assist in making decisions and solving problems related to their caregiving roles

· Work with Fraser Health and other agencies to facilitate formal education and training programs, classes, or workshops that promote health and well-being, including recruiting volunteer leaders

· Resource information for respite care and supplemental services

· Reporting, data collection, outcomes, and evaluations

· Develop and maintain caregiver library of resources

· Provide public education on caregiver’s support services and identify individuals needing caregiver support

· Provide clients with home visits as necessary





· Attend training as required

· Perform any other job-related duties as required

JOB & QUALIFICATION REQUIREMENTS:

· Social Services Diploma, or BA Social Work.

· Lived experience as an informal Caregiver.

· Must have good verbal and written communication skills, be confident in public speaking, and have interpersonal skills.

· Ability to oversee multiple volunteers.

· Interviewing and counseling skills.

· Ability to work in a dynamic team-based environment, collaborating frequently.

· Must be able to manage several client cases at once

· Knowledge of social services or human services issues, especially dealing with older adults and family caregivers

· Valid Class 5 Driver’s License

· Ability to solve problems and prioritize potential solutions; and able to prioritize multiple on-going tasks.

· Keen interest in caregiving, seniors’ issues, and health care.

· Ability to follow oral and written instructions

· Knowledge of local community social services and health care systems.

· Ability to obtain a clean criminal record check

ROLE RESPONSIBILITIES:

1. Caregiver Support

· Respond to family caregiver inquirers, offering emotional support as well as practical assistance · Suggest local caregiver support options

· Assist caregivers with problem solving and creating action plans (when appropriate)

· Record, track, and report data as per requirements

· Prepare ‘call/contact log’ statistics.

Caregiver Support Group Coordination

· Recruit, train, and coach volunteers while providing group sessions · Host and provide group sessions for volunteers

· Attending training in person or online to help build skills

1. Community Engagement

· Initiate and respond to community-based requests for information/education on AAHA, Family & Friend Caregiver Support Program, and caregiver issues. · Develop new and enhance existing connections with community organizations and local health care professionals to increase awareness of the Family & Friend Caregiver Support Program. Aim to align the Family & Friend Caregiver Support Program with other local services/programs.

· Create outreach and education opportunities for the Family & Friend Caregiver Support Program to be represented at community events (i.e. Health fairs, lunch & learn presentations, seniors’ events)

· Coordinate the ongoing recruitment, training and support of volunteer family and friend caregivers.

1. Volunteer Coordination

· Recruit, train, and coach volunteers for a variety of volunteer program roles. · Implement Volunteer Recognition, both formally and informally in collaboration with AAHA teams.

· Maintain confidential volunteer records.

1. Communications: Social Media/Website

· Work collaboratively with AAHA to promote caregiver-related programs, training sessions, presentations, events, topics, and volunteer recruitment.
