Position Summary
Reporting to the Program Manager, the Clerk, Regional Kidney Care Program provides clerical and reception support to facilitate the day-to-day operations of the Home Dialysis and Hemodialysis programs. The Clerk serves as a primary point of contact for patients, families, and internal and external partners, providing clerical support across Home Dialysis and Hemodialysis services and may be assigned to work in multiple program areas throughout the Regional Kidney Care Program based on operational needs. The role coordinates communication, appointment scheduling, patient registration, and documentation while supporting patient flow and contributing to a safe, organized, and patient-centered environment in accordance with Hospital policies and procedures.
Key Responsibilities
- Provides excellent customer service and serves as a primary point of contact for patients, families, and internal and external partners, responding to inquiries in a qualified and timely manner.
- Answers and directs telephone calls and electronic communications, referring clinical matters to the appropriate member of the interdisciplinary team.
- Registers patients and accurately obtains, validates, and updates patient demographic and health coverage information in accordance with organizational standards.
- Coordinates patient appointments, clinics, and scheduling activities in accordance with Ontario Renal Network (ORN) guidelines and program requirements.
- Communicates appointment details, schedule changes, and other relevant information to patients and members of the interdisciplinary team in a timely manner.
- Maintains accurate patient records and performs data entry within organizational and provincial information systems, ensuring data quality, completeness, and confidentiality.
- Identifies and addresses data quality concerns in a timely manner and participates in data quality initiatives to support program and organizational objectives.
- Prepares, distributes, retrieves, and files patient records, reports, and program documentation in accordance with privacy legislation and Hospital policies.
- Maintains the clerical work area in an organized manner and assists with processing mail, photocopying, scanning, faxing, and other clerical support functions as required.
- Maintains effective working relationships with patients, families, physicians, team members, and external partners to support coordinated, patient-centered care.
- Adheres to Hospital policies related to confidentiality,
privacy legislation, professional conduct, infection prevention and control, and workplace safety.
- Performs other duties as assigned by the Program Manager or designate
Qualifications
Education
- Ontario Secondary School Diploma or equivalent is required.
- Successful completion of a Medical Terminology course required.
- Completion of relevant post-secondary program is preferred (such as Medical Office Administration).
Experience
- Minimum of one (1) year experience working in a clerical/administrative role required.
- Preference given to candidates with previous acute care experience.
Skills and Traits
- Keyboarding skills; minimum of 40 words per minute required.
- Must be able to meet the physical and emotional responsibilities of the role, including standing and walking for extended periods, frequent and repetitive pushing, pulling, bending, and lifting up to 50 pounds, as well as safely operating equipment.
- Ability to meet the physical demands of the position, as outlined in the Physical Demands Analysis (PDA).
- Demonstrates proficiency in written and verbal communication, with strong interpersonal and organizational skills.
- Provides excellent customer service and contributes positively to teamwork and collaborative working relationships.
- Effectively prioritizes workload, adapts to changing priorities, and remains composed in fast-paced or stressful environments.
