COMMUNITY HEALTH WORKER

PERMANENT FULLTIME – FTE 1.0

37.5 HOURS/WEEK The Community Health Worker (CHW) is an integral member of the Inter-professional Primary Care Team (IPCT) at the Community Care City of Kawartha Lakes Community Health Centre (CCCKL CHC). The CHW strengthens connections between clients, healthcare providers, and community resources to ensure individuals and families receive coordinated and equitable care.

Working collaboratively with the Health Promoter, Wellness Program Coordinator, and other IPCT members, the CHW assists clients in navigating health and social systems, promotes equitable access to services, and supports the overall well-being of the community.

Key Responsibilities

- Assist clients in navigating the healthcare system, including scheduling appointments, completing forms, and connecting with appropriate health and social services.
- Work with the IPCT to identify clients requiring additional support and follow up to ensure coordinated care.
- Provide one-on-one assistance to help clients understand and manage chronic conditions, preventive care, and treatment plans.
- Conduct proactive outreach to engage individuals and families who may experience barriers to accessing primary care.
- Promote awareness of CHC programs and community-based health and wellness resources.
- Build trusting relationships with clients through culturally sensitive, trauma-informed, and strengths-based approaches.
- Collaborate with the Wellness team to deliver community health education and engagement initiatives.
- Contribute to outreach strategies that complement existing programs and health promotion priorities of the CHC and advocate for clients by identifying and addressing barriers to care.
- Collect and maintain accurate data on outreach activities, client interactions, and identified community needs.

KEY REQUIREMENTS AND COMPETENCIES

- A college diploma or undergraduate degree in Community Health, Public Health, Social Work, Social Service Work, Community Development, or a related discipline.




- Ability to work collaboratively within an interdisciplinary team and independently as needed.
- Experience working in community health, social services, or primary care environments is preferred.
- Additional training in systems navigation, cultural safety, or trauma-informed practice is considered an asset.
- Mental Health First Aid or Applied Suicide Intervention Skills Training (ASIST) is an asset.

COMPENSATION: The wage range for this exciting opportunity is $26.89 – $29.44 per hour.Selected candidates will be appropriately placed on the organizations wage grid based on skill, knowledge and experience.*
