Licensed Practical Nurse (LPN) / Business Development Lead

Job Summary

We are seeking a compassionate, motivated, and relationship-focused Licensed Practical Nurse (LPN) to join our team. This unique role combines clinical oversight with community outreach and business development to support the growth of our home care services.

As the LPN, you will provide nursing oversight and care for every client, ensuring care is safe, personalized, and responsive to changing needs, even when day-to-day care is provided by caregivers. You will work closely with clients, families, caregivers, and healthcare professionals to develop and monitor individualized care plans that help clients remain comfortable, supported, and independent in their own homes.

In addition to your clinical responsibilities, you will build and maintain strong relationships with referral partners, healthcare providers, community organizations, and prospective clients. You will represent the organization within the community, promote our services, and help identify opportunities to expand our client base.

Key Responsibilities:

Clinical Responsibilities

- Provide nursing oversight for home care clients.
- Complete client assessments and reassessments.
- Develop, implement, and update individualized care plans.
- Monitor client health status and communicate changes with families, caregivers, physicians, and other healthcare providers.
- Provide clinical guidance and support to caregivers to ensure safe, high-quality care.
- Maintain accurate documentation in accordance with professional and regulatory standards.
- Promote client safety, comfort, independence, and quality of life.

Business Development Responsibilities

- Build relationships with physicians, hospitals, rehabilitation facilities, community agencies, and other referral sources.
- Conduct community outreach to increase awareness of our home care services.




- Meet with prospective clients and families to discuss services and assess their care needs.
- Represent the organization at networking events, community presentations, and healthcare meetings.
- Identify opportunities to grow the business through strategic partnerships and referral development.
- Collaborate with leadership to achieve client growth and business development goals.
- Deliver exceptional customer service while fostering long-term relationships with clients and community partners.

Qualifications

- Current Licensed Practical Nurse (LPN) license in valuable standing.
- Experience in home care, community nursing, or senior care preferred.
- Experience in business development, sales, community outreach, or healthcare marketing is an asset.
- Strong clinical assessment and care planning skills.
- Excellent interpersonal, communication, and relationship-building abilities.
- Self-motivated with strong organizational and time management skills.
- Comfortable working independently and within a collaborative team environment.
- Valid driver's license and reliable transportation for community visits.
- Footcare designation is an asset.
