Position Profile: Provide direct support to families and individuals living with a cognitive disability such as FASD and/or acquired Brain Injury. Facilitate the development of basic living skills, advocacy skills, and support interdependent living or independent living based on the spectrum of the disability.
Hours of Work: Monday – Friday 8:00am – 4:00pm
Location: Head Office – Wellness Centre
Position Reports to: Wellness Coordinator
Major Duties and Job Responsibilities:
· Work through the intake process with individuals, opening file, collecting information and determining areas of strength and challenges to support individuals and their families.
· Using a team approach support individuals with FASD and similar cognitive disabilities with a philosophy of building on strengths.
· Work with individuals and families in goal setting to reduce challenges and to promote interdependent and/or independent living.
· Develop and implement support plans for persons with FASD and similar cognitive disabilities.
· Carry out assessments of individuals on an ongoing basis to determine needs and progress.
· Educate and create community awareness of the necessity to put the client’s needs first to support a greater equitable quality of life.
· Work in partnership with other vested organizations to meet the needs of individuals with FASD and similar cognitive disabilities.
· Provide one on one support to accomplish all the necessary activities to create stability and a personally satisfying quality of life.
· Promote community inclusion and interdependence for persons with FASD and similar cognitive disabilities.
· Support parent(s) with children in parenting skills and day to day parenting requirements.
· Support parent(s) with children in care. Provide supervised visits if required.
· Support individuals with everyday life-skills, such as shopping and budgeting.
· Support individuals with justice matters, attend court, or other judicial appointments with the individual.
· Support individuals and families to secure housing.
· Support individuals to secure employment-working with potential employers to accommodate the individual needs where possible.
· Support individuals to achieve higher learning based on the spectrum of their disability.
· Support the health of individuals by working with them to make appointments, with family physician, mental health, addictions, psychiatrist and psychologist appointments.
· Support individuals and families to live healthy lifestyles, through nutrition and recreation.
· Support group work within the Wellness Center responding to client needs.
· Take an active role in team planning for scheduled quarterly activities within the centre
· Engage community services and internal knowledge into group programming and individualized Mentor supports
· Other duties as required.
Job Type: Full-time
Pay: $22.00-$25.00 per hour
