We are seeking a candidate for the position of Community Health Nurse (RN) to join the Primary Health Care team based at our Oakwood-Vaughan site. The Community Health Nurse (RN) will coordinate the daily operations of the Oakwood-Vaughan site and its ongoing Primary Care program. The ideal candidate for this position will have a strong background in clinical primary care, experience in health teaching, advocacy and health promotion.
Job-Specific Responsibilities:
- Oversee staff schedules and coordinate with the superviser to ensure primary care coverage at the site.
- Provision of primary care and wellness assessments across the lifespan including immunization, chronic disease monitoring, triage and assessment over the phone, point of care testing, venipuncture, counselling and assessment, and care of minor episodic illnesses in collaboration with physicians, nurse practitioners, and allied health professionals.
- Health teaching and advocacy for individuals within a harm reduction framework.
- Working collaboratively within a multidisciplinary team.
- Participate in external committees or meetings as required.
- Performance of administrative tasks.
- Coordinates with the supervisor in ordering and maintenance of clinical supplies, equipment, resources.
- Participate in the orientation and onboarding of new staff.
Job-Specific Qualifications:
- RN with a current CNO membership and an undergraduate degree in nursing, or a combination of educational preparation and relevant experience.
- Current RNAO membership with malpractice coverage is an asset.
- Strong clinical assessment skills, documentation and communication skills.
- Solid understanding of and experience with primary care and the multi-disciplinary nature of community health issues.
- Familiarity with the social, economic and cultural conditions of the geographic area.
- Experience serving Unison’s priority populations (seniors with complex needs, people with mental health & addiction issues, LGBTQ2IA+, and youth facing barriers).
- Knowledge of program/service development, implementation, monitoring and evaluation.
- Effective communication, interpersonal, organizational and administrative skills, including computer literacy skills specific to job requirements.
- Knowledge of a second (or more) language and culture is an asset.
- Ability to work flexible hours.
