The Renfrew Victoria Hospital (RVH) and St. Francis Memorial Hospital (SFMH) are fully accredited facilities; RVH is 45 minutes west of Ottawa and SFMH is in beautiful Barry’s Bay, about an hour west of Renfrew. **Come join us! We are hiring a Permanent, Full-Time Integrated Social Worker/Discharge Planner.
Pay Grid: $43.89 - $53.34/hour** We are currently recruiting for a permanent full time Social Worker/Discharge planner to provide support and discharge planning to our inpatient unit at SFMH along with our two ALIGN programs (ALIGN – Aging at Home through Linked, Integrated & Coordinated Care),
one located in each community of Renfrew and Barry’s Bay.
Working as a member of the clinical team reporting to the Manager of the ALIGN program, the Social Worker/Discharge planner assists the provider (physician, NP), patient and family, and interdisciplinary team with the implementation and planning of the arrangements for the level of care, whether it be in another hospital, a care facility or in the patient’s home. In addition, they will identify and facilitate transition of appropriate clients to the ALIGN programs of Renfrew and Barry’s Bay. This position requires travel between hospitals and could involve client facing visits in the home.

As Social Worker/Discharge Planner, experience with individualized discharge care plans and familiarization with care needs of the elderly and vulnerable populations will be considered strong assets.

Qualifications Qualifications
- Minimum Bachelor’s Degree in Social Work (Master’s Degree of Social Work an asset).
- Certificate of Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required
- Minimum 2 years’ experience working in a related position preferred
- Highly effective oral, written, and interpersonal communication skills
- Experience providing a range of evidence-based interventions, including solution-focused therapy, cognitive-behavioral therapy, dialectical behavior therapy, and motivational interviewing
- Knowledge and experience in substance use and harm reduction
- A satisfactory and current Vulnerable Sector Screening check
- Valid First Aid & CPR certificate or willingness to obtain.
- Must hold a valid driver’s license for travel within the region when required
