As a Nurse Practitioner, you will deliver comprehensive patient care within the RN-EC scope. This role requires a minimum of five years of health care experience and a commitment to culturally safe practices. You will also conduct home visits and collaborate with an interdisciplinary team to ensure holistic approaches in your care.
Key Responsibilities:
- Provide direct patient care across various health needs
- Conduct home visits for patients unable to access the clinic
- Maintain accurate patient records in PSSuites EMR
- Collaborate with specialists and interdisciplinary teams
- Educate patients on preventive care and chronic disease management
Requirements:
- Nurse Practitioner registration in good standing
- Minimum 5 years in direct care roles
- Knowledge of Indigenous community health issues
- Strong communication and interpersonal skills
- Transparent vulnerable sectors check required
