face‑to‑face meetings and one‑on‑ones.Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.Maintain the knowledge, skills, and attitudes to provide secure, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.What You BringCurrent RPN registration with the College of Nurses of Ontario and entitled to practice without restrictionsMinimum of 5 years’ experience as a practicing Registered Practical NurseComprehensive patient health assessment skills are requiredPossess an understanding of the principles of community nursing practiceProven experience leading and managing a teamKnowledge of clinical documentation standards and practice guidelines is requiredExcellent organizational, interpersonal, and teaching skillsCommitment to professional development in clinical practice that is relevant to home health careEffective critical thinking and problem‑solving skillsMust possess a valid driver’s license and access to reliable transportationAble to provide a current, clear Vulnerable Sector Screening (VSS)C.N.A.
