In this role, you will assess client needs and provide support for mental health housing placement, working within a team to promote independent living. Key Responsibilities:
• Conduct comprehensive mental health housing assessments
• Facilitate referrals for home support services

Requirements:
• Required Master's Degree in a Health Sciences field
• At least two years of relevant experience
