Provide essential support as a Care Assistant for post-hospital clients in Timmins, Ontario. Help clients transition home by managing care plans and addressing their immediate needs.
This permanent full time role emphasizes individualized care and community engagement. You will assist clients in understanding discharge instructions, coordinate transportation, and perform necessary home-management tasks. Your role will also ensure clients receive the companionship and resources they require to prevent readmission and promote safety at home.
Key Responsibilities:
• Arrange transportation and drive clients home safely
• Develop and implement tailored care plans
• Conduct errands for client needs, including nutrition and medical supplies




• Evaluate safety risks in the home environment
• Document observations and communicate with coordinators
Requirements:
• One to two years of post-secondary education in social services or related fields
• At least three years of relevant experience
• Bilingual in English and French is necessary
