Nova Scotia Health seeks a Primary Health Care Coordinator to advance PHC priorities within Chronic Disease Management and the Department of Family Practice portfolios. You will collaborate with leaders, physicians and teams to plan, coordinate, facilitate and evaluate PHC projects, building relationships with staff and partner agencies.

This role offers a hybrid/work-from-home model and requires travel to Central Zone facilities. A valid driver’s license and reliable transportation are required.
