Parkwood Institute is looking for skilled Hospitalists to join its Complex Continuing Care Program. This role involves direct patient care for up to 35 individuals, emphasizing continuity, emergency interventions, and cooperation with healthcare staff and community physicians. You will play a pivotal role in managing care needs on a part-time basis in a supportive setting.
Key Responsibilities:
• Provide clinical care for complex continuing patients
• Participate in scheduled after-hours call duties
• Conduct routine patient rounds and support urgent care




• Collaborate with fellow physicians for effective care delivery
• Participate in relevant hospital committees and discussions
Requirements:
• Must be eligible for membership with the Ontario College of Physicians
• Preferably hold CCFP or equivalent certification
• Experience in Family Medicine or relevant practice area
• Availability for call duty and vacation support
• One-year term commitment with an option for extension
