In your position as an Older Adult Outreach Nurse, you will work directly with elderly individuals facing mental health challenges and concurrent disorders. Responsibilities include conducting psychogeriatric assessments, treatment planning, and collaborating with healthcare providers. You will also coordinate community resources and provide education to clients and families.
Key Responsibilities:
• Conduct assessments and facilitate treatment plans
• Engage in crisis intervention and direct care services
• Provide consultation to families and practitioners




• Deliver educational programs for community resources
• Maintain cooperative relationships with clients and caregivers
Requirements:
• Registered Nurse or Registered Psychiatric Nurse in BC
• 3 years of experience in geriatric mental health and addictions
• Valid B.C. Driver’s License for travel
