The Community Health

Nurse will provide essential services in WRHA Community Health Services, focusing on health promotion, chronic disease management, and support for underserved populations. Your work will involve collaborating with an interdisciplinary team to meet diverse healthcare needs and facilitate a healthy living setting for clients. Key Responsibilities:
Deliver comprehensive care across the community
Create tailored health plans for individuals and families
Work closely with physicians and other health specialists
Provide preventive care and health education
Assist with resource navigation for client support Requirements:
Two years’ experience in community or primary healthcare
RN education program graduate required
Must maintain registration with CRNM
Experience in mental health counseling beneficial
