In this full time role, you will assess individual behaviours, collaborate with caregivers, and implement effective behavioural interventions.
The position requires extensive interaction with hospital staff and families, ensuring a seamless transition of patients from hospital to community-based care. Your expertise will be vital in dealing with challenges related to cognitive impairments, ensuring optimal patient support. Key Responsibilities:
Conduct behavioural assessments as per best practice guidelines
Collaborate with caregivers to identify behaviour triggers
Implement and document tailored behavioural interventions
Provide training to staff involved in patient care
Develop individualized transitional care plans Requirements:
Community college diploma in healthcare, Bachelor's preferred
One year experience with seniors in acute settings
Certified in gerontology preferred
Robust communication and interpersonal skills
