This position on the Pathways Assertive Outreach Team involves implementing evidence-based strategies for individuals facing addiction and mental health challenges. Your role is crucial in assessing needs and creating tailored support plans for participants. Work collaboratively with a transdisciplinary team in a energetic and supportive environment.
Key Responsibilities:
• Assess program participants and develop care plans
• Oversee adaptive equipment for enhanced daily living
• Facilitate group programs supporting participant engagement
• Conduct outreach services within participants' home settings
• Maintain meticulous documentation as per agency standards
Requirements:
• Bachelor's degree in occupational therapy
• ACOT registration mandatory
• Experience addressing complex mental health needs
• Valid Class 5 driver’s license and clean driving record
• Commitment to team culture and ongoing feedback
