Key Responsibilities:
• Conduct psychosocial assessments for patients and families
• Facilitate Advance Care Planning and Goals of Care conversations




• Provide counseling and emotional support to patients
• Lead group work and consult within the team
• Teach and engage in career development initiatives

Requirements:
