Compensation: $ 32.84-$ 41.35

Job Summary
The Licensed PracticalNurse (LPN) at Phoenix Society works collaboratively with the interprofessionalteam to provide health care services to clients with physical and/or mentalhealth, substance dependence, concurrent disorder and chronic disease. The LPNis responsible for planning, implementing and evaluating nursing care within aninterprofessional clinical framework in accordance with Standards of Practicefor the British Columbia College of Nurses and Midwives (BCCNM), and consistentwith the mission, vision and values of Phoenix Society. This is a7.5 hrs/week position that can be adaptable in days and hours 1-2 days a week.

Responsibilities
Client Services

Assistphysicians and other team members in the performance of various examinations,treatments and diagnostic procedures such as physical examinations

Perform avariety of nursing procedures such as taking blood pressures, heights andweights, and urine testing as directed

Communicatepatient information to designated supervisor, physicians, and other appropriatemembers of the team

Assistdesignated supervisor in caring for clients with unpredictable outcomes orwhere the acuity or complexity of care requires an advanced level of knowledgeand skill beyond the scope of practice for Licensed Practical Nurse

Contributeto the development and modification of individualized care plans by identifyingclient priorities/plans and suggesting revisions of the nursing care plan asrequired for clients

Monitorstatus of clients in relation to anticipated outcomes. Evaluate theeffectiveness of nursing interventions by comparing actual outcomes toanticipated outcomes. Report unanticipated changes and concerns to designatedsupervisor

Assess andidentify clients’ actual and potential strengths and limitations, includingphysiological, psychological, social-cultural and spiritual needs. Provideemotional support as needed

Work incollaboration with the treatment team to assist the client with their programrequirements by methods such as monitoring program requirements andmotivational interviewing

Review themedication administration record for each client and check with the designatedsupervisor or the physician regarding any concerns

Delivermedications consistent with the written care plan, physician orders andstandards of BCCNM

Store allclient medications according to policies and procedures

Establishtherapeutic relationships with clients

Administration

Maintaincomplete and accurate computerized and paper records through methods such as tracking results and consultation documentation, and inputting data anddocumentation appropriately on the client’s health record

Occupational Health & Safety





Maintainan awareness of current Occupation Health & Safety guidelines and standards

Useappropriate supplies and equipment in accordance with organizational standardsand safe care requirements

Handle,store, and dispose of workplace hazardous materials and sharps in accordancewith established procedures

Identifyand report unsafe practice/professional conduct in accordance with professionalstandards and organizational policies

Participatein incident investigations as required

Respondto incidents and provide nursing care such as, injuries to staff or visitors,Code Blue or Code White emergencies involving patients, hazardous and/or minorspills, or aggressive and/or disruptive behaviors.Providefirst aid to injured persons up to Occupational First Aid Level 1 standards

CompleteFirst Aid reports and Incident Report forms

Assessindividual client risk factors and incorporate risk mitigation strategies intoclients’ care plans in order to optimize safety and security for staff, otherclients and the general public

Qualifications

Post-secondarydiploma (e.g., 2 to 3 years) in nursing or a related field

Current practicing registration as a Registered Nurse orRegistered Psychiatric Nurse with the British Columbia College or Nurses andMidwives (BCCNM)

1to 3 years of experience providing trauma-informedacute or community health nursing care/services to patients who have acombination of physical, psychiatric and/or concurrent substance abuse/misusedisorders, or equivalent

Satisfactory completion of a criminal records check withvulnerable sector screening is required

Satisfactory completion of Tuberculosis Screening form is required

Satisfactory completion of Employee Immunization Record form isrequired

First Aid 1 and Current CPR certificate isrequired

Satisfactory completion of Nonviolent crisis intervention(NVCI)

Validdriver’s license is required

Skills & Knowledge

Excellentknowledge of community health nursingtheory including mental health and addictions, communicable diseases, primarycare and practice within a patient/family-centered approach

Excellent knowledge ofBCCNM's Standards for Nursing Practice

Excellent knowledge ofthe BC Mental Health Act, other facilities and community resources

Goodknowledge of facilitation,mediation, and conflict resolution techniques

Good knowledge ofcomputer literacy to operate a computerized patient care information system andword processing, spreadsheet, Internet and e-mail software

Strongcase management, teaching/training, clinical support and consultation skills

Stronginterpersonal skills and ability to communicate effectively in writing andverbally in English
