In this vital position, you will work collaboratively on an interprofessional team delivering individualized care. This temporary full time position operates on a rotating schedule of 12-hour shifts.

Key Responsibilities:
• Deliver individualized hospice and palliative care plans
• Assess and manage pain and symptoms for residents
• Collaborate with families to provide psychosocial support




• Participate in care planning with interdisciplinary teams
• Maintain accurate documentation of care provided

Requirements:
• Registered member in good standing with College of Nurses of Ontario
• BCLS certification and palliative care course completion preferred
• 1-2 years of recent palliative care experience required
• Two Step TB test and Vulnerable Sector Check
• Strong communication skills with a passion for end-of-life care
