What you will do

Conduct urgent Occupational Therapy assessments to prevent hospital admissions.





Assess functional ability and environmental factors within patients’ homes.

Implement strategies and equipment to safely maintain independence.

Collaborate with multidisciplinary team (MDT) colleagues for crisis intervention.

Support safe discharge pathways to reduce avoidable delays in care transfer.

Coordinate with health, social care, and voluntary services for community support.

Develop person-centred plans to maximise function and mitigate admission risks.

Monitor outcomes and adjust interventions to support community recovery.

Maintain detailed clinical records and contribute to performance targets.

Requirements

Degree in Occupational Therapy.

HCPC Registration.

Prior NHS Occupational Therapy experience.

Post-qualification Occupational Therapy experience.

Community and Rapid Response experience preferred.

Skills in falls assessment and risk management.

Experience in admission avoidance and discharge support pathways.Strong clinical reasoning and decision-making abilities.

Ability to work autonomously in community settings.

Expertise in multidisciplinary team collaboration.

Experience in Rapid Response Teams.
