Key Responsibilities:
• Perform assessments and diagnoses based on standard practices
• Consult with Nurse Practitioners on complex client issues
• Document and participate in chart reviews and case conferences




• Support health promotion and disease prevention initiatives
• Participate in team meetings and organizational goals

Requirements:
• Medical degree from a recognized university
• General license from the College of Physicians and Surgeons of Ontario
• CFPC Certification or equivalent experience
• Experience with diverse and complex populations
• Proficient in MS Office and electronic medical records
