This term position falls under the Department of Opportunities and Social Development. You will work closely with individuals needing varying levels of support, conducting assessments and implementing support plans. The role requires collaboration with service providers to ensure effective service delivery while adhering to health and social service principles.

Key Responsibilities:
• Conduct intake assessments and determine service needs
• Coordinate and implement personalized support plans
• Monitor ongoing case management and discharge planning




• Collaborate with service providers for effective program delivery
• Travel to various locations for client assessments

Requirements:
• Licensed Skilled in Nursing, Social Work, or related field
• Minimum five years of relevant experience required
• Excellent assessment and negotiation skills essential
• Valid driver’s license for site visits
• Strong understanding of community resources and support systems
