Summary of Duties:
As a member of the Sunnybrook Emergency Department, the Nurse Clinician (GEM) works closely with the ED One Team supporting protected early discharges from the Emergency Department. Providing assessment, consultation and community linkage for frail seniors in collaboration with other team members, the Nurse Clinician (GEM) will:
Provide consultation to health care professionals and frail seniors regarding geriatric health issues
Perform expedited assessments of the care needs, coping strengths, family/community support structure of frail seniors with complex geriatric health problems; and support decision making on safe early discharge from the ED, or hospital admission
For patients who are safe to be discharged home, support discharge planning including linkage to community resources and supports
Complete follow up phone calls for frail seniors discharged overnight when indicated by ED PCM/ED One Team Lead
Qualifications/Skills:
Bachelor's degree in Nursing required.
Master's degree in Nursing or Gerontology is an asset.
Current College of Nurses of Ontario Certificate of Competence; current B.C.L.S.
Experience working in community health care
Working knowledge of community resources, LHIN, and RGP
Experience in specialized geriatric programs is an asset.
Additional training and/or experience in Gerontology is required
Knowledge of Comprehensive Geriatric Assessment (CGA)
Excellent oral and written communication skills
Demonstrated ability to function on an interdisciplinary team
Knowledge and skills in functional assessment and the assessment of physical and mental illness in the elderly
Excellent organizational skills, problem solving and time management skills
Ability to adapt to change, innovate, and be flexible in an ever-changing environment
