Join De dwa da dehs nye>s Aboriginal Health Centre as a Nurse Practitioner in Hamilton, providing culturally protected care to Indigenous communities. Support primary care services directly to patients and enhance health outcomes.

As a Nurse Practitioner, you will deliver comprehensive patient care within the RN-EC scope. This role requires a minimum of five years of health care experience and a commitment to culturally safe practices. You will also conduct home visits and collaborate with an interdisciplinary team to ensure holistic approaches in your care.

Key Responsibilities:
• Provide direct patient care across various health needs
• Conduct home visits for patients unable to access the clinic
• Maintain accurate patient records in PSSuites EMR




• Collaborate with specialists and interdisciplinary teams
• Educate patients on preventive care and chronic disease management

Requirements:
• Nurse Practitioner registration in good standing
• Minimum 5 years in direct care roles
• Knowledge of Indigenous community health issues
• Strong communication and interpersonal skills
• Clear vulnerable sectors check required
