Based in Sudbury, Ontario, Compass is the Lead Agency for child & youth mental health services for the districts of Sudbury and Manitoulin. Reporting to the Client Services Manager, the Transition Support Worker will support youth aged 12-25 by providing case management, system navigation, and individualized or group-based supports to help them achieve their goals. Working as part of a multidisciplinary team, the role connects youth and their families with health, social, and community resources to promote continuity of care and improve access to integrated mental health, substance use, and wellness services.
RESPONSABILITIES:
· Create and maintain a developmentally appropriate, non-judgmental, non-stigmatizing, safe, and welcoming hub atmosphere.
· Provide support that is consistent with YWHO values to youth, their families and caregivers as part of a multi-disciplinary team.
· Support a seamless experience of care for youth and families, including the transition between various service pathways internal to the Hub Network (Skills and Well-being, Community & Social Support) as well as linking youth to resources in the community.
· Accompanying youth to appointments and provide warm transfer supports to youth referred to services both in the Hub and service providers in the community.
· In collaboration with youth, support the use of YWHO standardized measures to provide mental health and substance use services, monitor progress, and improve youth outcomes (focus on measurement-based care).
· Provide services and supports to youth and their families in a way that is consistent with the values of the YWHO model.
· Participate in YWHO Provincial Community of Practice.
· Provide access to services both in person, remote (off site) or virtually (online, phone) as needed.
· Promote and encourage youth engagement and positive youth development in care planning and establish goals by fostering therapeutic relationships.
· Coordinate and oversee bus ticket access for youth, including receiving and reviewing requests from Youth Wellness Hub staff and partners, issuing tickets as appropriate, and maintaining an accurate central log of all bus ticket requests and distributions in accordance with established procedures.
· Develop strong relationship with community services to build referral pathways for young people looking for specific services and supports such as housing, education, financial supports, employment, legal, basic needs, mental health, substance use, and physical health.
· Maintain accurate youth records including case note documentation, progress reports and all other functions within the electronic records management system.
QUALIFICATIONS:
· Successful completion of a Child & Youth Care or Child and Youth Worker College Diploma
· Valid First Aid/CPR will be required.
· CPI will be required.
· 2-3 years of experience in healthcare or the social services sectors.
· 2-3 experience working with children, youth, families and caregivers.
· Comprehensive knowledge of issues related to substance use, concurrent disorders, mental health, trauma and recovery.
· Knowledge of the impact of social/cultural/economic factors on the determinants of health.
· Knowledge of community resources and services considered an asset.
· Proficiency working in a computerized environment, including Microsoft Office applications (Word and Excel).
· Demonstrated ability to maintain strict confidentiality and handle sensitive information with professionalism and discretion.
· Demonstrated ability to assess priorities, analyse and solve problems
· Strong organisational skills and ability to meet deadlines
· Ability to develop a thorough understanding of the Agency's programs, services, organizational structure, and operations.
· Ability to work collaboratively with other experienced professionals including school boards, colleagues of other health and social service agencies, local politicians, etc.
· Demonstrated ability to work both independently and collaboratively within a multidisciplinary team.
· Excellent organizational and written communication skills and the ability to work effectively in a fast-paced environment are essential.
· Excellent interpersonal skills in connecting/building relationships with youth, family/caregivers and service partners.
· Knowledge of approaches that promote health equity, including anti-oppressive and anti-racist practice, 2SLGBTQIA+ inclusion, community engagement, cultural safety, trauma-informed care, harm reduction, and human rights and accessibility frameworks.
· Excellent oral and written communication skills in English is required. Excellent oral and written communication skills in French are considered an asset.
