The Care Coordinator will work with Individuals referred into the program by Acute care. The Plan Coordinator oversees the development, implementation, coordination, and monitoring of the care plan once for individuals who have transitioned back to the community. This role focuses on ensuring continuity of care through case management. The goal of this position is to support older adults to transition home from ALC spaces to age safely in place by coordinating necessary supports.

QUALIFICATIONS, SKILLS & EXPERIENCE

- Post-secondary education in Social Work, Human Services, Gerontology, Nursing, etc.
- Required to be a Registered Social Worker with the ACSW
- Knowledge of issues impacting older adults including complex needs and transitions from ALC settings and resources available
- Demonstrated ability to monitor service delivery, manage competing priorities, and assess progress towards outcomes
- Minimum of 3 years of experience in service planning,



coordination or case management
- Police Information Check including Vulnerable Sector is required
- Vehicle required; valid class 5 driver’s license and $2 million auto liability insurance
- Experience working with diverse cultures and ethnocultural communities is an asset

This position is a permanent, full-time in-person (35 hours/week) position. The salary range for this position is $47,500 - $59,060 plus 100% Employer paid Health & Dental plan, and 5% matched Pension plan. Salary considerations are based on an assessment of the successful candidate’s education, skills, previous experience and other factors. Sage Seniors Association offers an outstanding work life balance with 3 weeks of vacation to start, a 35-hour workweek, and generous sick leave accruals. Sage also observes 13 statutory holidays in the calendar year.Social Workers are re-imbursed for their annual ACSW registration fee as well as Social Worker Liability Insurance premiums. This position will require some evenings and weekends; shift differential will be applied.
