DDC has exciting opportunities for compassionate and dedicated Home Support Workers (HSW) to join our dynamic staffing pool. We are seeking caring, reliable, and skilled professionals who are committed to promoting the health, comfort, dignity, and independence of individuals receiving care in home care, community, residential, and healthcare settings.

You are committed to maintaining client dignity, respecting privacy and confidentiality, and supporting individuals with compassion, professionalism, and empathy.

This role requires individuals who are confident and experienced in providing care to clients with complex medical conditions, cognitive impairments, dementia, mental health concerns, mobility limitations, and responsive or unpredictable behaviours while ensuring a safe and supportive environment.

Key Responsibilities

The successful candidate will be responsible for the following on a day-to-day basis:

Personal Care & Activities of Daily Living

- Provide safe, compassionate, and individualized personal care in accordance with established care plans, organizational policies, and professional standards.
- Assist clients with activities of daily living (ADLs), including:
- Bathing and personal hygiene
- Dressing and grooming
- Toileting and continence care
- Mobility, transfers, and positioning
- Feeding and nutritional support
- Promote client dignity, independence, privacy, and quality of life while encouraging participation in daily activities whenever possible.

Health Monitoring & Clinical Support

- Observe, monitor, and report changes in clients' physical, emotional, and cognitive conditions to the supervising nurse or healthcare team.
- Assist with routine health monitoring including vital signs, intake and output, weight monitoring, and other delegated care tasks as permitted.
- Support medication reminders or assistance in accordance with organizational policy and provincial regulations.
- Follow infection prevention and control (IPAC) practices, including proper use of personal protective equipment (PPE).

Dementia, Behavioural & Cognitive Support

- Provide compassionate care to individuals living with dementia,



Alzheimer's disease, cognitive impairment, mental health challenges, or responsive behaviours.
- Utilize person-centered, trauma-informed, and gentle approaches to reduce anxiety and promote emotional well-being.
- Apply approved de-escalation and behaviour support techniques while maintaining a calm, respectful, and professional presence.
- Monitor behavioural changes and communicate concerns promptly to the healthcare team.

Mobility & Safety

- Assist clients with safe transfers, ambulation, lifts, repositioning, and the use of mobility equipment including mechanical lifts where required.
- Maintain a safe environment by identifying potential hazards and implementing fall prevention strategies.
- Ensure client comfort while promoting independence and mobility whenever appropriate.

Companionship & Emotional Support

- Foster meaningful relationships through compassionate communication and active listening.
- Encourage participation in recreational, therapeutic, and social activities that promote emotional, physical, and mental well-being.
- Support family members by communicating professionally and respectfully while maintaining client confidentiality.

Documentation & Professional Practice

- Accurately document care provided, observations, incidents, and changes in client condition in accordance with organizational, legal, and regulatory requirements.
- Maintain effective communication with nurses, supervisors, physicians, allied healthcare professionals, and other team members to ensure continuity of care.
- Adhere to all occupational health and safety policies, infection control protocols, privacy legislation, and organizational standards.

Emergency Response

- Respond appropriately to medical emergencies and client incidents using approved First Aid, CPR, and emergency response procedures.
- Report incidents, accidents, injuries,



and safety concerns promptly following organizational policies.

Requirements to Be Successful in This Role

To ensure the highest standard of care and client safety, applicants must meet the following requirements:

Mandatory Qualifications (one of the following):

- Relevant Care Certificate
- Nursing Student (completed clinical placements)
- Internationally Educated Nurse (IEN) with appropriate documentation
- Equivalent recognized healthcare support worker credential

Experience & Skills

- Minimum 1 year of experience providing direct personal care in hospitals, long-term care, retirement homes, community care, or home care settings.
- Experience supporting clients with dementia, Alzheimer's disease, palliative care, rehabilitation, disabilities, or complex medical needs is considered an asset.
- Solid understanding of person-centered care, infection prevention and control, protected lifting and transferring techniques, and client safety.
- Excellent communication, documentation, observation, and interpersonal skills.
- Ability to remain calm, professional, and compassionate in rapid-paced healthcare environments.
- Physical ability to perform repetitive lifting, transfers, repositioning, standing, walking, and other physically demanding tasks.

- Standard First Aid & CPR (Level C)
- Safe Patient Handling / Mechanical Lift Training
- Gentle Persuasive Approach (GPA) (Asset)
- Non-Violent Crisis Intervention (CPI) (Asset)
- WHMIS
- Infection Prevention & Control (IPAC) Training
- AODA (Accessibility for Ontarians with Disabilities Act)
- 4 Steps Health & Safety

Additional Requirements

- Vulnerable Sector Check (issued within the last 6 months)
- TB Test or Chest X-Ray
- Proof of immunization as required by healthcare facilities
- Valid Government-issued Photo Identification
- Reliable transportation is considered an asset
- Ability to work days, evenings, nights, weekends, and holidays as required
