Job Description: We are hiring foe a premium home care agency in GTA providing premium medical and non-medical care. We are seeking a detail-oriented, licensed nurse (RN or RPN) to join our team as a Care Coordinator. In this dual-responsibility role, you will review custom client care plans to ensure safety and clinical compliance, and strategically manage our staff scheduling logistics. You will be responsible for matching clients with the best-suited Personal Support Workers (PSWs) based on skills, location, availability, language preferences, and personality matches, ensuring the highest continuity of care. You will oversee the entire scheduling lifecycle, publishing shifts and delivering clear care instructions to field staff before every client visit. Requirements & Qualifications: - Active CNO Registration: Must be a Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario. - Experience:



Minimum 3–10 years of experience as a Care Coordinator or Scheduler in a home care or healthcare setting. - Industry Knowledge: Strong knowledge of home care operations and PSW scheduling. - Software Skills: Experience using ShiftCare, AlayaCare, or similar home care software is preferred. - Core Skills: Excellent organizational, communication, and problem-solving skills. - Adaptability: Ability to manage multiple priorities in a fast-paced environment while delivering exceptional client service. - Compliance: Knowledge of Ontario home care standards and privacy requirements (PHIPA) is an asset. - Background Check: Explicit Vulnerable Sector Check (VSC).
