Role Overview

We're seeking Primary Care NPs to provide on‐site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.

What You'll Do

- Routine & urgent assessments; chronic disease management
- Post‐discharge follow‐ups, medication reconciliation, labs/diagnostics
- Coordinate with facility staff, specialists, families/POAs
- Same‐day documentation; CMS/regulatory compliance

Schedule & Setting

Full‐time position with a structured weekday schedule. Providers deliver on‐site care across Stockton, Modesto, and Amador with strategically clustered routes, so you spend more time with patients and less time on the road.

Requirements

- Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
- Geriatrics or facility‐based experience preferred; EMR proficiency

Benefits

- Annual compensation range: $140,000-$175,000 (based on experience, schedule, and patient volume)
