This contract Nurse Practitioner (NP) role is a vital addition to the clinical team at West Park LTC. As a Nurse Practitioner, you’ll help bridge gaps in healthcare delivery, prevent unnecessary hospital transfers, and lead the way in evidence-based care planning for geriatric and medically complex residents.

Responsibilities

- Conducting comprehensive health assessments and medical histories
- Diagnosing, treating, and managing acute and chronic conditions
- Prescribing medications and managing polypharmacy
- Participating in care conferences and communicating with families
- Providing clinical leadership and support to the nursing and PSW teams
- Educating staff on disease prevention, infection control, and best practices




- Supporting hospital diversion initiatives and continuity of care
- Collaborating with physicians, specialists, allied health, and administration
- Participating in emergency response and on‑call rotations (as agreed upon)

Requirements

We’re looking for someone who brings both clinical excellence and a deep commitment to compassionate senior care. The ideal candidate will have:

- A valid Nurse Practitioner license in Ontario and active CNO registration
- An OHIP billing number and comfort operating as an independent contractor
- 3+ years of NP experience (preferably in long‑term care or geriatrics)
- A background in complex care, dementia, and chronic disease management
- A collaborative, educational mindset to guide team learning and development
- Strong clinical judgement, communication, and family engagement skills
- Flexibility to work full‑time or part‑time and openness to on‑call support
