In this position, reporting to the Care Manager, you will assume the role of Case Manager. Your main responsibilities involve assessing client needs and collaborating with the interdisciplinary team. You will continually modify care plans and ensure timely referrals to stakeholders while also providing direct care as necessary.
Key Responsibilities:
• Coordinate community-based quality care for clients
• Assess patient needs and develop individualized care plans
• Collaborate with healthcare teams to ensure effective service




• Facilitate timely referrals to necessary stakeholders
• Provide direct nursing care when required
Requirements:
• Active registration with the College of Registered Nurses of Alberta
• Graduation from an accredited nursing education program
• Current Basic Cardiac Life Support certification
• Access to a reliable vehicle for work travel
• Minimum one year of community nursing experience preferred
