Parkdale Queen West Community Health Centre (PQWCHC) is a dynamic, community-based health care organization that has historically served mid‐west downtown Toronto, and now provides outreach services across the entire city. PQWCHC offers a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming, and is committed to serving people who face barriers to health care.The Homelessness and Addiction Recovery Treatment (HART) Hub Nurse Practitioner is responsible for providing health care and treatment for clients who use substances. The NP is part of the HART Hub services and will work closely with the mental health, substance use care (continuum of treatment, recovery, harm reduction), and health promotion teams to ensure cross‐team and cross‐site collaboration.Reporting to Primary Care management, the NP will participate in larger HART Hub partnerships led by the Centre, which may include, but are not limited to, CAMH, LOFT, and the Neighbourhood Group to participate in resource sharing and knowledge exchange practices.General ResponsibilitiesPerforms clinical tasks, including history taking, ordering routine diagnostic tests, making diagnoses and plans of care, and providing addictions treatment.Provides brief counselling and support to clients to manage their mental health concerns,



substance use needs or other difficulties and makes referrals to internal or external programs and services.Identifies trends or issues related to the health status of priority groups and uses this information in planning and advocacy work.Establishes clinics at partnering agencies in conjunction with other clinical and non‐clinical staff.Performs routine outreach with internal Centre programs/leads as appropriate.Provides primary care for the duration of the treatment period and refers clients to appropriate wraparound services, including allied health services, for continued care post‐discharge.Maintains timely, high‐quality documentation in client/patient files through PQWCHC's EMR to meet the requirements of the funders, the Centre, and regulatory bodies (i.E., College of Nurses of Ontario).Leads the development of substance use care protocols as required.Participates in the design, implementation, and evaluation of research studies in consultation with the Management team and other staff.Participates in evaluation, communities of practice, presentations, and academic requests for information sharing.QualificationsCurrent registration as a Registered Nurse (Extended Class) with the College of Nurses of Ontario, and RNAO membership.3 to 5 years nursing experience in a community setting, public health setting,



and/or primary care.Experience with street or outreach nursing is an asset.Knowledge and experience in mental health, substance use care, harm reduction and chronic disease.Strong oral and written communication skills.Solid problem‐solving and adaptive skills.Excellent decision‐making skills.Understanding of anti‐oppression principles and frameworks.High level of comfort with diverse populations; demonstrated ability and comfort in working with homeless populations, people who use drugs and people with mental health challenges.Able to work independently and as a team member within a multi‐disciplinary environment.Second language is an asset.Compensation$115,524 to $137,499 per annum plus 10% in lieu of benefits and holiday pay; 8% in lieu of vacation and HOOPP (Healthcare of Ontario Pension Plan) option.Successful candidate will be required to provide a police vulnerable sector check as a condition of employment and will need to be fully vaccinated with COVID‐19 (2 doses, plus booster within 6–12 months) 14 days before starting employment.PQWCHC is committed to the principles of access and equity.
