This job posting is for a permanent part‑time position (not locum) to augment a family practice with a long‑term care (LTC) panel to provide challenging, diverse, and impactful work caring for the frail elderly.
Responsibilities and Expectations
- Serve as a practitioner at Sunrise of Killarney, a 105‑bed long‑term care home.
- Assist with resident caseload and support the current LTCI practitioners.
- Participate in LTCI incentive payments, mentorship and care home onboarding, LTC‑specific events and projects, and the after‑hours care program.
- Commit to formal LTCI best practice expectations:
regular proactive visits; on‑site medication reviews; attendance at care conferences; involvement in a 24/7 on‑call rota; completed documentation in charts.
- Collaborate with an interdisciplinary team (physicians, nurse practitioners, nurses, allied health, and care home leadership) to provide holistic resident‑centered care.
- Develop skills and confidence in long‑term care through a supportive community of practice.
