By working at The Care Company, you will be part of a growing organization that is helping individuals and families thrive at home.Key ResponsibilitiesComplete initial in-person home assessments within 24 hours of referral, including functional assessments, mobility evaluations, equipment needs, and ADL/IADL analysisProvide follow-up visits (30-minute or 15-minute sessions) focusing on goal progression, safety, mobility, environmental modifications, and skill buildingDeliver client-centered care aligned with HCCSS/OH@H expectations, including timely communication and adherence to care pathwaysDevelop and implement individualized treatment plans based on client goals, safety considerations, and clinical best practicesEducate and coach clients, families, and caregivers on functional strategies, equipment use, home safety, and risk mitigationCoordinate with interdisciplinary providers (PTs, PSWs, nurses, case managers) to support seamless care transitionsDocument all assessments, treatment plans, progress, and communications in a clear, timely, and clinically compliant mannerRecommend, trial, and prescribe assistive devices and home modifications, collaborating with vendors or contractors when neededSupport in ordering supplies through OH@H vendorsMonitor client progress and modify care plans as appropriate to support meaningful, measurable outcomesMaintain communication with the office regarding caseload availability, scheduling updates, and urgent visit readinessUphold The Care Company's commitment to secure, high-quality, ethical, and compassionate community careSchedulingFlexible schedule, mainly Monday-Friday,
with occasional Saturday/Sunday availability required, based on client needAble to complete an initial in-person home assessment within 24 hours of receiving a visit request within geographic areaTherapists are assigned geographical zones. Clients can be located within Central Region (may include Richmond Hill, Markham, Newmarket, Simcoe, Keswick)Ability to manage scheduling independently and communicate promptly with the office regarding availabilityTravel between client homes required; access to a reliable vehicle is an assetQualification and Experience RequirementsExperience providing home and community care, specifically with HCCSS and/or OH@H (Ontario Health at Home) clients strongly preferred; will also consider experience in community OT or hospital discharge/transition planning experienceRegistered and in good standing with the College of Occupational Therapists of Ontario (COTO)ADP authorizers are preferredDemonstrated skill in providing functional assessments, equipment recommendations, Equipment and supply ordering through OH@H vendors, safety evaluations, and ADL retraining in the home environmentStrong understanding of community discharge planning, fall prevention, mobility supports, and caregiver trainingSkilled at electronic documentationExperience coordinating with interdisciplinary teams and communicating with case managersAbility to work independently in the community setting and produce clear, timely clinical documentationValid driver's license and access to reliable transportation (or ability to travel efficiently within designated area)BenefitsCompensation$150 per initial assessment, including supply ordering and funding applications$75 per in-person follow-up visit$50 per virtual follow-up visitOther BenefitsPPE providedMileage ReimbursementVisits scheduled to minimize travelEducational Training OpportunitiesEmployee Recognition ProgramHealth and Wellness ProgramStaff & service provider eventsOpportunity for Group Health and Dental Benefits #J-18808-Ljbffr
