and continence knowledgeCoordinates/collaborates with interprofessional team across the continuum of care to reduce duplication of services, client anxiety, and costProgram Development/LeadershipSupports quality health care through teaching, mentoring, program development and quality monitoringOptimizes client outcomes through review of Quality improvement (QI) reports to prioritize needIdentify nursing trends and issues related to specialized wound ostomy and continence managementAssist in the development of information tools and educational initiatives for care providersRemain updated on current evidences related to wound, ostomy, and continence best practicesPromote and participate in research-based practiceIdentify potential research initiativesPromote and participate in research projectsSubmit case studies for publicationParticipate in conferences through poster presentations or oral abstractsSupport WORNs and wound care champions in development of posters and case studies for conference presentationsParticipates with professional nursing associations to develop and enhance the delivery of health care for CanadiansCollaborate in local Home and Community Care Support Services Ontario Health at Home (OH@H) initiatives/working groups as requiredQUALIFICATIONS/REQUIREMENTS:Must have completed Nurse Specialized in Wound, Ostomy, & Continence Canada (NSWOCC) WOC-EP program. Other equivalent programs or post-secondary wound care programs or has completed advanced knowledge programs including:The University of Western Ontario Masters of Clinical Science in Wound Healing The University of Toronto/CAWC International Interdisciplinary Wound Care Course (IIWCC) PLUS a mentorship/preceptorship such as through the RNAO Advanced Practice Clinical Fellowship (APCF)



The University of Toronto Masters of Science of Community Health in Wound Healing The Masters of Science in Wound Healing and Tissue Repair, Cardiff, Wales,Or is a Master's prepared Advanced Practice Nurse (APN) who has received formal education in skin and wound care.Must be an RN with current Certificate of Competence with the College of Nurses of Ontario.This position will require travel around Huron County.Available to work FT or PT.Flexibility and a commitment to advancing wound, ostomy and continence care at SE Health Care is required.CNA certification credential WOCC(C) preferred.A willingness to participate in further education programs/courses to remain current.Adoption of the SE model for wound care delivery through mentorship of WORN/K2P WCRN.A vehicle and valid driver's license and a willingness to travel are required.Excellent communication and teaching skills are required.A comprehensive knowledge of community health care is preferred.Demonstrated teaching skills and expertise in preparing effective teaching tools and methods are preferred, if not a willingness to develop in that area.The ability to use a computer word processing, power point, email, and source information on the internet is preferred.About SE HealthSE Health is a not-for-profit social enterprise advancing health with heart.
