Expected Hours: Full-time hours (1.0 FTE)Zone: Western ZoneSpecialization: Family PracticeType of Remuneration: Longitudinal Family MedicineIncentive Programs Available: YesCompany DescriptionMiddleton & Area Family Health Centre is seeking two physicians to join their team of nine family medicine physicians (two are relocating), 3-4 residents, two nurse practitioners, two family practice nurses, and a licensed practical nurse, as well as a part time social worker, dietitian and occupational therapist. All team members work collaboratively to provide patient care. While each MD and NP have their own roster, each clinician provides 1⁄2 day a week of same-day fit-in appointments covering the entire clinic patient population. Some minor procedures are performed onsite and others completed at the nearby Soldiers Memorial Hospital (SMH).The clinic uses a MedAccess electronic medical record. Each clinician does their own day-today- clinic billing and billings for activities outside the clinic can be provided by the administration staff (in-patient shadow billing, nursing home billing, etc).It is highly encouraged that physicians contribute to the community at large outside the clinic in some way. Currently, the physicians work as part of the inpatient rotation at SMH providing inpatient coverage in a hospitalist-type model, covering all inpatients x 1 week block q 6-7 weeks. Other options include: same day/next day evening and weekend clinics located in Berwick and Wolfville, Emergency Medicine at SMH; nursing homes, addictions medicine, and other areas of personal interest (leadership, surgical assists at Valley Regional Hospital, etc). If teaching is an interest, Middleton also trains family medicine residents from the Dalhousie Annapolis Valley site; they currently have three-four residents. Physicians also intermittently teach undergraduate medical learners. All learners have a dedicated area for study and conversation at the practice. The health‐care centre is in a beautiful 12,600 sq ft building that has 26 exam rooms, consultation and conference rooms as well as a large waiting area and administration space.

It is located adjacent to Soldiers Memorial Hospital which has a 18‐bed medical unit along with 24‐bed transitional care unit, 9‐bed alternate level of care unit and 25‐bed nursing home unit. The catchment area is approximately 40,000. Valley Regional Hospital is half an hour away in Kentville as well. If emergency medicine is of interest, you don't need additional training/ER certification to practice in the hospital's Emergency Department. The site offers physical and psychological safety due to the spacious design, privacy, calm décor, and beauty of the space. There is sufficient area for collaboration within the interdisciplinary team of physicians, nurses, social worker, dietitian and occupational therapist and administrative staff. This team truly embodies the ethos of collaboration, providing cross‐coverage for the entire clinic patient population and meeting weekly to review difficult cases. You can often find members of the team sharing time together in the large sunny lunchroom, which facilitates informal collaboration and development of shared community.Job DescriptionResponsibilitiesensure the delivery of comprehensive primary health care services to patients of all ages and stages of health or illnessbe accountable for appropriate workload and practicecontinuum of patient care with health care providers within your practice and/or communityprovide care and coordination of community-based care, secondary and tertiary level heath carebuild to capacity within the first two yearsoperate within the Bylaws and Rules and Regulations of Nova Scotia Health Authoritywork toward same day or next day access for the practice populationwork in collaboration with other team members in the practice to best suit the community's health needswork with other providers to provide after‐hours access and emergency coverage when requiredparticipate in quality improvement and evaluation activitiesutilize an electronic medical record (EMR) and record all relevant patient details in a provincially approved EMR which is accessible to all collaborating team membersExpected Hoursclinic offers after‐hours appointmentsphysicians work evenings on a one day per week rotationLocated near the centre of the Annapolis Valley, Middleton - otherwise known as "The Heart of the Valley" - is a scenic town with a population of approximately 2,000.
