develop and review care plans, and provide direct clinical services.Order and interpret diagnostic investigations, prescribe medications, and coordinate referrals to specialists and community resources.Educate clients on disease prevention, health promotion, and chronic disease management.Maintain accurate and timely documentation in the electronic medical record in accordance with skilled and organizational standards.Participate in team meetings, case conferences, quality improvement, and continuing professional development activities.Support clients experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.QualificationsDoctor of Medicine (MD) degree and current licensure in good standing with the College of Physicians and Surgeons of Ontario (CPSO).CCFP certification or eligibility with the College of Family Physicians of Canada.Current CMPA coverage.Minimum of two (2) years of experience practicing family medicine, ideally in a primary care or community health setting.Demonstrated experience or strong interest in working with one or more of PQCHC's priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and people experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.Experience in a community setting desirable.Demonstrated commitment to and knowledge of community-based health care.Technology proficiency – PS Suite EMR, Microsoft Office Suite.Flexible hours, including some evenings.Vehicle and license requirements – Class G.Excellent communication and interpersonal skills.Linguistic profile: Bilingual English/French is an asset. #J-18808-Ljbffr
