Key Responsibilities:
• Develop customized care plans in collaboration with the healthcare team
• Participate in clinical discussions and teamwork
• Provide holistic patient-centered care and support
• Operate under the supervision of the Integrated Care Program Supervisor
• Travel within the designated region to patient homes
Requirements:
• Registered in valuable standing with the College of Nurses of Ontario
• Minimum of two years nursing experience, ideally in palliative care
• Proven experience handling palliative care cases
• Valid Ontario driver’s license and willingness to travel
• CPR and First Aid Certifications up to date
