In this position within PHC's Urban Health & Substance Use Program, you'll work across various clinics, including the Lighthouse Substance Use Care Clinic. By leveraging your clinical judgement and teamwork skills, you'll support patients on their journey towards recovery and better health outcomes.

Key Responsibilities:
• Assess and document patient health status and needs
• Collaborate to develop and assess care plans




• Facilitate learning by conducting patient education
• Advocate for patients’ rights and dignity
• Participate in ongoing staff training and development

Requirements:
• Registration as a Registered Nurse or Psychiatric Nurse
• Robust analytical and decision-making capabilities
• Experience in interdisciplinary team settings
• Ability to take initiative in crisis situations
• Comfort with the physical demands of nursing tasks
