In this part time position, you will engage with patients at St. Clair Medical Clinic, Yonge Davisville Health Clinic, and Forest Hill Family Health Centre. Your role involves comprehensive mental health interventions and advocacy to ensure patient access to primary care services. Collaborate with a diverse team to develop and implement care plans that prioritize patients’ needs.

Key Responsibilities:
• Assist in patient attachment to primary healthcare
• Collaborate effectively with interdisciplinary teams
• Conduct independent psychosocial assessments




• Provide tailored mental health support and counselling
• Participate in clinical case conferences and initiatives

Requirements:
• Master’s degree in Social Work (MSW) mandatory
• Registered with Ontario College of Social Workers
• Experience in community and primary care
• Solid time management and communication skills
• Knowledge of EMRs and community resources
