You will execute care plans, provide in-person and virtual support to patients and families, coordinate referrals, and assist with discharge planning under supervision of a Licensed Social Worker.
Ideal candidates will hold a Diploma in Social Service Work, be in positive standing with the Ontario College, have computer skills, and a valid driver’s license for community visits.
