Schedules are subject to change to meet operational needs.Developmental Services' TAY home is a supported living program that provides transitional aged youth (TAY) with developmental disabilities, supporting their transition from child welfare and development of life skills for independent living.ResponsibilitiesNeeds AnalysisCollaborate with individuals with developmental disabilities to develop goals, plans, and strategies that align with their unique needs and enhance independence.Conduct effective needs analysis in collaboration with the individual to identify unique needs.Service DeliverySupport the delivery of services per goal plans to foster transition to independent living.Assist senior DSWs in locating e‐programs, services, housing, and employment opportunities aligned with goal plans.Work collaboratively with TAY staff to develop strategies and implement therapeutic programming.Ensure personal and physical care, crisis response, counseling, and emotional support are provided in an effective, trauma‐informed manner.Program DevelopmentSupport development of programs within and outside the home, implement life‐skills facilitation enabling independent community integration.Ensure program plans promote independent living, reflect high learning levels,



and are developed collaboratively with the Supervisor.Facilitate life‐skills programming individually or in groups, based on needs, goals, and support plans, within timelines and standards.Participate in assessing long‐term service needs and program evaluation.Community OutingsProvide supports for individuals to actively participate in community activities and recreational outings safely.Plan outings, appointments, and activities meeting safety protocols and promoting social inclusion.Crisis ManagementReduce safety incidents through strategies that reinforce positive behavior and identify behaviors before, during, and after incidents, collecting objective data as required.Apply Individual's Behavior Support Plans consistently during crises or challenging behavior.Program and Administrative DutiesProduce professional written materials and correspondence that reflect agency philosophy and privacy standards.Monitor finances for individuals as required, following policies and practices.QualificationsEducation / ExperiencePost‐secondary diploma/degree in Social Services or Developmental Services, or a degree/diploma in a related area with at least two years of experience in developmental services.Valid CPR/First Aid and Crisis Prevention Institute (CPI)
