Position Summary Registered Practical Nurse (RPN) or Registered Nurse (RN) to support our Adult Visiting Nurse program in the East Toronto region. The role requires evening bi‑weekly weekend shifts (Saturday and Sunday). Responsibilities include delivering advanced clinical care such as wound care, medication administration, VACC, IV start, PICC and central line management, ostomy and drain care, chemotherapy management, and mental health support. Key Responsibilities Assess, monitor, and develop care planning and care coordination. Evaluate care plan interventions through ongoing reassessment and evaluation results. Ensure patient care quality by identifying and addressing factors that affect practice autonomy. Demonstrate professionalism, leadership, judgement, and accountability in client and family interactions. Support safe client care through development of knowledge and skills in continuous quality improvement. Inform nursing supervisor of client activities, concerns, or changes in condition and submit appropriate documentation. Maintain documentation standards per the College of Nurses of Ontario. Participate in a standby/on‑call rotation for answering calls as needed. Attend team meetings during stated availability. Qualifications Registered and in valuable standing with the College of Nurses of Ontario (CNO). Preferred membership of professional association (RNAO/WeRPN). RPN diploma or RN diploma/degree from a recognized university or college. Valid First Aid or CPR certification.



Moderate level of wound care or venipuncture knowledge and experience (asset). Acute care and medical‑surgical nursing experience (asset). Knowledge of community nursing (asset). Effective written and verbal communication and documentation skills. Reliable attendance, punctuality, and availability. Commitment to lifelong learning and critical thinking. Valid Ontario driver’s license (G) and reliable vehicle. Compensation & Benefits Pay Rate: $26.60 – $41.10 per visit (placement on salary range per Collective Agreement).
