OverviewWe are looking for a Family physician with advanced training in Care of the Elderly to join our team in Fraser East, affiliated with Fraser Health Authority.ResponsibilitiesProvide care to home health and homebound seniors in the Chilliwack community, including home visits and liaison with the home and community care team and the client's family physician.Collaborate in an interdisciplinary setting; may contribute to transitional care (hospital to home)
