In addition to make full use of your wound and ostomy care knowledge, each day you will use your skills in promoting self-management by patients and leading the interdisciplinary team to achieve optimal patient outcomes. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.What The Role InvolvesUsing your knowledge of assessment, prevention and interventions regarding wound infections and ostomy careCare for patients with consuming wounds, ostomy, continence, provide lower leg assessment including ABPIscritically appraising the wound care literature and apply research evidence to practiceProviding classifications and indications, appropriate utilization to reduce visit frequency and obtain desired outcomesPracticing conservative sharp wound debridement (CSWD) and use of other debridement methodsProviding edema management including bandaging based on complete lower leg assessmentMonitoring criteria for adjunctive therapy for complex or recalcitrant woundsSupporting our patient’s safety by practicing according to policy and standardsUsing your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the Ontario Health atHome referral and other sourcesFunctioning as a member of a nursing team who are accountable for the nursing care it providesDeveloping appropriate time-specific,
